Specialised Support Coordination

Specialised support coordination is intended for participants with complex circumstances, significant risks or multiple service systems involved in their support.

 

Our specialised support coordinators work with participants and their support networks to identify barriers, reduce risks and establish more stable and effective support arrangements.

 

Specialised support coordination may include:

• Understanding the participant’s NDIS plan and funded supports

• Identifying complex barriers affecting service delivery

• Coordinating disability, health, housing and community services

• Supporting the participant through periods of crisis or instability

• Establishing clear roles and responsibilities between providers

• Strengthening communication across the participant’s support team

• Identifying and managing service gaps

• Assisting with risk management and contingency planning

• Supporting complex transitions

• Building the participant’s confidence in managing supports

• Preparing reports and evidence relating to support needs

• Helping establish sustainable long-term support arrangements

 

Our approach remains participant-led. We support participants to understand their choices, make informed decisions and remain actively involved in how their supports are coordinated.
