Ideal Health Care provides professional community nursing and high-intensity personal support for participants with more complex health and daily care requirements.

 

Our services are delivered with a strong focus on safety, dignity, clinical oversight and consistency. Supports are tailored to the participant’s assessed needs, care plans, risk assessments and NDIS funding.

 

Where required, our nurses and appropriately trained support workers collaborate with participants, families, carers, health professionals and other providers to promote coordinated and safe care.

 

Community nursing care may include:

• Nursing assessments and ongoing monitoring

• Wound care and dressing changes

• Medication support and medication-related monitoring

• Health observations and clinical monitoring

• Continence assessments and support

High-intensity daily personal activities

Some participants require specialised assistance with high-intensity daily personal activities. These supports are provided by workers who have completed relevant training and have been assessed as competent for the participant’s specific needs.

 

Depending on the participant’s individual care plan, support may relate to:

• Complex bowel care

• Enteral feeding

• Dysphagia and mealtime management

• Urinary catheter support

• Subcutaneous injections

• Diabetes management

• Complex wound care

• Seizure-related support

• Respiratory support

Before services begin, we work to ensure appropriate documentation, clinical instructions, risk assessments, training and emergency procedures are in place.
