Life Stages and Transitions

Changes in living arrangements, relationships, health, education or employment can create additional support needs.

We assist participants to plan for and navigate important life stages and transitions with greater confidence and stability.

 

Transition support may include:

• Moving into a new home or living arrangement

• Transitioning into supported independent living

• Leaving hospital and returning home

• Starting or finishing school

• Moving into further education

• Preparing for employment

• Changes in family or informal support arrangements

• Adjusting to changes in health or disability

• Building new routines following a major change

• Connecting with appropriate community and mainstream services

• Developing practical plans for increased independence

• Coordinating supports during periods of change

 

We work collaboratively with the participant and their chosen support network to make transitions as organised, safe and positive as possible.

 

Employment and Higher Education Support

Employment and education can provide independence, confidence, skills and a sense of purpose.

We support participants to access and maintain suitable employment or higher education opportunities based on their interests, abilities and goals.

 

Support may include:

• Exploring employment or study goals

• Identifying strengths, interests and suitable opportunities

• Preparing for interviews or enrolment

• Developing workplace or study routines

• Building communication and organisational skills

• Support to travel to work, training or education

• Assistance to understand workplace expectations

• Support to maintain employment or study participation

• Building confidence in professional and educational environments

• Connecting with employment, training and educational services

• Developing strategies to manage disability-related barriers

 

Supports are tailored to the participant and are intended to complement, rather than replace, responsibilities held by employers, education providers or mainstream employment services.
